29 CFR 1926.453(b) (2) (v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: This violation occurred on or about September 13, 2012, at a jobsite located at 4417 South Lancaster Road in Dallas, Texas 75216, at least one (1) employee on the southwest side of the commercial building was installing Tyvek paper from an aerial lift without the use of a body belt and lanyard.
