29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Worksite located at 5924 SW 12th St. Oklahoma City, OK 73128. On or about March 11, 2015 an employee was observed working from an aerial lift without the use of a fall protection lanyard. The employee was exposed to a fall hazard of approximately 25 feet. William Beltron was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 941283, citation number 1, item number 1 and was affirmed as a final order on 03/12/2014, with respect to a workplace located at 4417 South Lancaster Road. Dallas, TX 75216.
