29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating:     (a) Outside receiving area, a Toyota propane-powered forklift was being operated with a propane fuel cylinder that was not securely mounted.  The cylinder mounting clamp was missing, exposing the employees to a fire and explosion hazard.

The electrical junction box did not have a cover installed, exposing the employees to an electrical hazard.    (b) East side of the warehouse, an electrical junction box for compressor #2 had live electrical wires carrying 220 volts AC that were not protected from accidental contact. The electrical junction box did not have the cover properly installed, exposing the employees to an electrical hazard.
