29 CFR 1910.212(a)(3)(ii): Points of operation of machines were not guarded to prevent employees from having any part of their body in the danger zone during operating cycles.         Steel Room.  Machine operator(s) were exposed to getting their fingers and hands into the danger zones on the following machine due to inadequate point of operation guarding:     a)  Peddimax 110/140, Hydraulic Universal Steelworker, Punching Unit.  Operator(s) were exposed to getting their fingers caught in the danger zone between the stripper head/punching ram and the top of the piece part.         b)  Peddimax 110/140, Hydraulic Steelworker, Notching Unit.   Operator(s) were exposed to getting their fingers and hands caught in the danger zone between the notching unit tooling/ram and the top of the piece part.          Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour.    Tool Room, Next to Gantry Crane.  Oxygen cylinders in storage were not separated from acetylene cylinders by a minimum of 20 feet or noncumbustible barrier at least 5 feet high having a fire resistance rating of at least one half hour. Two oxygen cylinders were stored together with two acetylene cylinders used to fuel torches.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
