29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. Line 2, Rocheleau Blow Mold Machine- Employees were not protected from nip points and moving parts which could cause laceration, crushing, and amputation injuries where the take away at the left side of the machine was not guarded.  b. Line 3, Rocheleau Blow Mold Machine- Employees were not protected from nip points and moving parts which could cause laceration, crushing, and amputation injuries where employees were reaching into the take away to clear jams.  c. Line 5, Uniloy Blow Mold Machine- Employees were not protected from moving parts at the point of operation which could cause crushing and laceration injuries where the swing arm was not guarded.  d. Line 6, Velocity Blow Mold Machine- Employees were not protected from nip points and moving parts which could cause crushing, laceration, and amputation injuries where employees were reaching into the take away and de-tabber to clear jams.
