Engine Plant - On or about September 18, 2013, the following load handling fixtures were permitted to be hoisted over personnel. 1. Chain Sling (Crankcase Fabrication, Bay #1) 2. Lifting Beam (Engine Assembly line, Bay #5) The employees were thereby exposed to struck by hazards associated with suspended loads.
