29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a) 104 Commerce Avenue, Greencastle, PA; East Side of Roof - The Safety Monitor was helping to the lay the rubber roof where a safety monitoring system was in place, thereby exposing roofers to an approximate 30 foot fall to the ground below, on or about September 20, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone. a) 104 Commerce Avenue, Greencastle Pa; West Side of Roof - Employees installing coping at the edge of a flat roof were observed without fall protection and were exposed to an approximate 30 foot fall hazard to the ground below, on or about September 20, 2013. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records. Bonded Applicators, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 314241761, citation number 2, item number 1, and was affirmed as a final order on October 27, 2010, with respect to a workplace located at East High Street, Gettysburg, PA 17325.
