29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:   a) East side of residence located at 12160 Charlotte Drive, Alpharetta, Georgia: An employee was exposed to fall hazards while on a walkway between two sections of the fabricated frame scaffold system consisted of a single 2-inch by 10-inch plank. The walkway was approximately 13-feet 6-inches above ground level.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:   a) East side of residence located at 12160 Charlotte Drive, Alphartetta, Georgia: The scaffold system was not supported on an adequate, firm, foundation. A number of the scaffold system legs were not bearing on base plates, but directly on mud sills.

29 CFR 1926.451(e)(6)(i): Integral prefabricated scaffold access frames were not specifically designed and constructed for use as ladder rungs:   a) East side of residence located at 12160 Charlotte Drive, Alpharetaa, Georgia: Employees were mounting and dismounting the working platforms of the scaffold system by climbing the supports of the scaffold end frames as if the supports had been designed and manufactured as ladder rungs.
