29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: On or about September 16, 2013, and at times prior thereto, the employer did not conduct hazard assessments for employees removing a glass window and reinstalling a glass window from a scaffold. Employees were exposed to the hazards of falls and being cut by/struck by the glass window.
