29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about September 19, 2013, and at times prior thereto, the employer did not assure 100% fall protection by use of a double line lanyard for employees conducting glazing activities. Employees were required to unhook single line lanyard from anchors to move to from one work area to another work area. Employees were exposed to the hazard of falls from heights up to nine (9) feet to lower level.
