29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:                                                                                                                                    a) Tech Manufacturing, LLC @ 45 Cooperative Way, Wright City, MO 63390: On and before September 20, 2013, the employer did not obtain manufacturer approval for an addition which affected the capacity and safety of the powered industrial trucks, in that a manufactured extended lifting boom, for the purpose of lifting and transporting third party tanks, was installed as a front-end attachment. This condition exposed employees in and around the facility to struck-by hazards.
