29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Left side of front roof: Employees were accessing a roof with a ladder that did not extend 3 feet above the platform, being exposed to a fall of approximately 11 feet, on or about 8/20/2013.  b) Left side of front wall: Employee was descending from a wooden platform with a ladder that did not extend 3 feet above the platform, being exposed to a fall of approximately 6 feet, on or about 8/20/2013.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Front roof: Employees without any means of fall protection were engaged in framing activities being exposed to a fall of approximately 11 feet onto a concrete sidewalk, on of about 8/20/13.   b) Interior of building, second floor: Employees without any means of fall protection were exposed to a fall of approximately 9 feet onto a concrete floor, on or about 8/20/13.   c) Right Front Framing: Employees without any means of fall protection were engaged in framing activities being exposed to a fall of approximately 11 feet onto a gravel surface, on of about 8/20/13.    The Tiger Builders LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 850443, citation number 1, item number 1A and was affirmed as a final order on 05/25/2013, with respect to a workplace located at 194 Broad St., Bloomfield NJ.
