Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to crushed-by or struck-by hazards: a) South Woodstock Baptist Church - On or about 9-25-13 - The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to the hazards of being struck-by or crushed-by heavy wooden trusses. The employer did not ensure through performing visual observation, specific instruction and providing appropriate information and training to the employees prior to and /or during installing wooden roof trusses. The trusses were not firmly and appropriately secured and braced to prevent them from displacement during installation. The majority of the wooden trusses were not diagonally braced and secured with 2 by 4 inch lumber. - On or about - 9-25-13 - employees were engaged in the installation of 13 wood trusses, 62 feet in length. The employees were working from scaffolding and standing in the web of the truss system installing metal bracing between each truss. The installation instructions supplied by the designer stated that 2 by 4 inch lumber was to be used for lateral and diagonal temporary bracing. The employer was using cut pieces of 3/4 inch particle board cut in short lengths. No vertical bracing was installed to the floor or ground. The live load imposed by the employees standing in the truss system, lack of diagonal bracing and not bracing ever four trusses, caused the 10 erected trusses to collapse on the four employees.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. a) South Woodstock Baptist Church - On or about 9-25-13 - Employees working on the second floor installing roof trusses were exposed to window openings with sill heights of 28 inches exposing employees to exterior fall hazards of 10 feet to the ground.
