29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 9/18/2014 employees at the Knights of Columbus roofing project were exposed to approximately 15' falls. Employees ladder did not extend at least three (3) feet above the landing.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about 9/18/2014 employees at the Knights of Columbus roofing project were exposed to approximately 15' falls. Employees were engaged in roofing activities without falls protection. Gagnon Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard: 29 CFR 1926.501 (b)(14), which was contained in OSHA inspection number 942697, citation number 01, item number 02 and was affirmed as a final order on 12/27/2013, with respect to a workplace located at 23 Roseland Park Road, Woodstock, CT 06281.
