29 CFR 1910.332(b)(1): Practices addressed in this standard. Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments. A HVAC Mechanic and Electrician who had performed energized electrical work consisting of voltage testing on voltages up to 480 Volts AC had not received specific electrical safety-related work practice training on when and what personal protective equipment to wear to protect himself against electric shock and arc flash.

29 CFR 1910.335(a)(1): Employees working in areas where there were potential electrical hazards were not provided with, and did not use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed: a. On or about September 19, 2013, in Building 261, an electrician was exposed to an electric shock and arc flash hazard while voltage testing lighting circuits, 277 Volt AC siingle phase for exit lights without wearing the appropriate personal protective equipment. b. On or about November 5, 2013, in Building 35 on a Air Handler Unit, a HVAC mechanic was exposed to an electric shock and arc flash hazard by not wearing the appropriate personal protective equipment when pushing in an energized 240 Volt AC blade that did not engage inside an electrical disconnect switchbox. Veterans Administration Medical Center, Chillicothe, OH was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 314652629, citation number 1, item number 18 and was affirmed as a final order on September 14, 2010, with respect to a workplace located at 17273 St. Rt. 104, Chillicothe, OH 45601.

29 CFR 1910.146(c)(2): If the workplace contains permit spaces, the employer shall inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces. NOTE: A sign reading DANGER -- PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign. In Building 261, Power Plant, the two pits, 10 ft. x 50 ft. by 10 ft., containing wood chips for the biomass boiler were not identified as a permit required confined spaces.
