29 CFR 1926.50(g): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. [44 FR 8577, Feb. 9, 1979; 44 FR 20940, Apr. 6, 1979, as amended at 49 FR 18295, Apr. 30, 1984; 58 FR 35084, June 30, 1993; 61 FR 5507, Feb. 13, 1996; 63 FR 33450, June 18, 1998]  a. At the jobsite where employees mixed and applied mortar containing Portland cement, no suitable eyewash facilities were present, thereby exposing employees to a contact hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  a. On the scaffold where employees were installing chicken wire to the facade of the building, guardrails were not installed on all open sides and ends of the scaffold, thereby exposing employees to a 16 foot fall hazard.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. On the west side of the building where employees were working from the overhang located at the front of the building, employees were not protected from a 10 foot, 6 inch fall, thereby being exposed to a fall hazard.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a. On the south side of the overhang where employees used a ladder to gain access to the upper landing, the ladder did not extend at least three feet above the landing surface, thereby exposing employees to a 8 foot fall hazard.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.  a. On the south side of the overhang where employees used a ladder to gain access to the upper landing, the ladder was damaged to the extent that a rung was broken in half and several were bent, thereby exposing employees to a 8 foot fall hazard.
