29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about October 4, 2013, the Pit Grinder #1 in the manufacturing area was not properly guarded. Employees were exposed to the rotating blades from a auger when operating the equipment. b) On or about October 4, 2013, the Wash Line #1 in the manufacturing area was not properly guarded. Employees were exposed to the rotating blades from a auger when operating the equipment. c) On or about October 4, 2013, the Dust Collector #1 in the manufacturing area was not properly guarded. Employees were exposed to the rotating blades when operating the equipment. d) On or about October 4, 2013, the Dust Collector #2 in the manufacturing area was not properly guarded. Employees were exposed to the rotating blades when operating the equipment. e) On or about October 4, 2013, the Rotating Magnet on Line #1 in the manufacturing area was not properly guarded. Employees were exposed to the rotating barrels in running nip point when operating the equipment. f) On or about October 4, 2013, the Roller on conveyor line #5 in the manufacturing area was not properly guarded. Employees were exposed to the rotating rollers underneath the conveyor causing a in running nip point when operating the equipment. g) On or about October 4, 2013, the Wash Line #1 in the manufacturing area was not properly guarded. Employees were exposed to the rotating barrels 1-3 in running nip point when operating the equipment.
