29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Job Site - Employees were not provided adequate hazard recognition training as evidenced by noted hazards such as but not limited to; cutting wooden framing member with a portable electric circular saw and not wearing eye protection, tearing down an exterior framed wall that was 10 feet in height and not wearing head protection, using a temporary electrical outlet that was not mounted, using portable electric tools connected to a temporary electrical service and not using a ground fault circuit interrupter, standing near a temporary electrical service panel without a cover and exposed to live electrical parts, exposed to fall hazards while removing demolition debris by standing at the edge of an open deck. and using a ladder to gain access to an upper landing that did not extend at least 3 feet above the landing, on or about 10/18/2013. AJP Builders, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.21(b)(2) which was contained in OSHA inspection number 921047, citation number 1, item number 4b, and was affirmed as a final order on 09/10/13, with respect to a workplace located at 3812 25th Place, NE Washington DC 20018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

AJP Builders, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 921047, citation number 1, item number 3, and was affirmed as a final order on 09/10/13, with respect to a workplace located at 3812 25th Place, NE Washington DC 20018. Abatement certification and documentation required within 10 days after abatement date.
