29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:          (a) Facility:  On or about October 18, 2013, the employer did not have a lockout/tagout program.  Two employees were inspecting the GMC vacuum truck to see what needed to be done to repair the vacuum system and the company did not have a lockout/tagout program.  An important part of this program is the unexpected start-up or activation of equipment.     (b) Facility:  On or about the end of October through November 2013, the employer continuously performed activities requiring energy control as recently from October 18, 2013 and thereafter.  The employer failed to maintain a lockout/tagout program.  Employees would do servicing and maintenance work on tri-axle dump trucks, snow removal trucks, a backhoe, two skid loaders, asphalt rollers and asphalt pavers.  An important part of this program is the unexpected start-up or activation of this equipment.
