29 CFR 1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving: (a) Facility, Shop located at 288 North Spring Street, Blairsville, PA: On or about April 23, 2015 the Craftsman 15 inch pedestal drill press was not anchored to the floor. This machine had been anchored in another location but was required to be moved because of its proximity to an electrical panel. The drill was moved to its current location approximately 1 month ago and was not anchored to prevent walking or moving. The employer, K. Dolan Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(b), which was contained in OSHA inspection number 943401, citation number 1, item number 6 and was affirmed as a final order on May 5, 2014, with respect to a workplace located at 288 North Spring Street, Blairsville, PA.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a non-combustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: (a) Facility, 228 North Spring Street, Mining Shop, Blairsville, PA: On or about April 23, 2015 oxygen and acetylene cylinders were stored together and not separated by a distance of 20 feet or a non-combustible barrier.
