29 CFR 1926.20(b)(2): The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer.    a)  Allison Apartments, N Maple Ave, Evesham:  The employer did not initiate a program for a competent person to conduct frequent and regular inspections of the jobsite, to identify hazards and/or stop the work in the event workers were placed in a hazardous condition. As such, an ACHA employee installing drip edge on the roof of a two story apartment building without any form of fall protection was exposed to a fall hazard of 17 feet from the roof's eave to the ground.  Observed on or about 10/17/2013.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  a)  Allison Apartments, N Maple Ave, Evesham:  An ACHA Construction Inc. employee installing drip edge on the roof of a two story apartment building without any form of fall protection was exposed to a fall hazard of 17 feet from the roof's eave to the ground.  Observed on or about 10/17/2013.      The employer,  ACHA Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 314230186, citation number 01, item number 001a and was affirmed as a final order on 01/11/2011, with respect to a workplace located at Ridley Brook Apartments, 111 McDade Blvd, Folsom PA.  The employer,  ACHA Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 314230756, citation number 01, item number 001a and was affirmed as a final order on 01/11/2011, with respect to a workplace located at 213 Rodmor Ave., Havertown, PA.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.    a)  Allison Apartments, N Maple Ave, Evesham:  An ACHA Construction Inc. employee installing drip edge on the roof of a two story apartment building without any form of fall protection was exposed to a fall hazard of 17 feet from the roof's eave to the ground.  The employer had not trained each employee exposed to fall hazards to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Observed on or about 10/17/2013.
