29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area and chemical-specific information was not always available through labels and material safety data sheets: (a) One Source Staffing, Inc. had not provided employees, assigned to Moldrite Products, Inc., located at N29 W22870 Marjean Lane, Waukesha, WI 53186, adequate training and information regarding the requirements of OSHA's Hazard Communication standard. For example, training and information provided by the temporary staffing company did not provide an overview or summary of OSHA's Hazard Communication standard. Also the temporary staffing company had not taken reasonable steps to assure that the client employer, Moldrite Products, Inc. was providing adequate site-specific training on the hazards of the chemicals in their work area. Chemicals used in the Molding Department of Moldrite Products, Inc. included but were not limited to Methylene Chloride (a suspected human carcinogen, with other potentially hazardous effects), Elastopor P1001U Isocyanate (containing MDI which can cause respiratory sensitization), RT-16911R Resin, Ease Release 205, and various paints. One Source Staffing, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 425552, citation number 1, item number 2 and was affirmed as a final order on July 19, 2012 , with respect to a workplace located at 10155 S. 57th Street, Franklin, WI 53132.
