29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator:     The employer does not provide an initial fit test prior to the initial use of the respirator to determine the employees ability to use a respirator. This violation occurred on January 3, 2014 at the establishment located at: 913 E. Pleasant Run Road Lancaster, TX 75146; where the employer did not a provide painter employee with an initial fit test prior to use of full face piece elastomeric respirator.

29 CFR 1910.1026(l)(2)(i)(A): The employer did not provide appropriate information and training for all employees exposed to chromium (VI), in that employees could not demonstrate knowledge of the contents of the Chromium (VI) standard, 29 CFR 1910.1026:     The employer does not provide appropriate information and training on the contents of the chromium (VI) standard. This violation occurred on or about January 3, 2014, in the spray painting booth, where an employee who had eye or skin exposure  to chromium (VI) was not provided appropriate information and training and could not demonstrate knowledge of the chromium standard, 29 CFR 1910.1026.
