29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a. Millcreek Estates Subdivision, Lot #17A, Lawrenceville, GA (Jobsite) - Employees were exposed to fall hazard. Employees were installing hardi-plank siding to the exterior of the newly constructed residential home while working on a ladder jack scaffold approximately 16 feet from ground level, without utilizing a means of fall protection, on or about October 23, 2013.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met (Construction Reference: 1926.59): a. Newbury Subdivision, Lot #4B, Regency Ave., Lawrenceville, GA (Jobsite) - Employees were exposed to hazardous material. Employees were required to cut cement fiber board siding "hardi-plank" to various sizes and length for the installation of the siding on the exterior sides of the newly residential home. A written hazard communication program had not been developed to promote the safe handling and use of the hazardous material silica, which is manufactured into the cement fiber board siding, on or about October 23, 2013.
