29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system.  a) 20 South 3rd Street, Roof - Employees installing a parapet and making roof repairs were exposed to a fall hazard of approximately fifty (50) feet without conventional or alternative methods of fall protection. 29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  a) Third floor - Employees working at the elevator shaft-way were exposed to a fall hazard of approximately twenty-five (25) feet without the use of a complete guardrail system.  Observed on or about October 21, 2013.  b) Fourth floor - Employees working at the elevator shaft-way were exposed to a fall hazard of approximately thirty-four (34) feet without the use of a complete guardrail system.  Observed on or about October 21, 2013.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed.  a) Access from landing to 4th floor - The portable A-frame ladder being used to access the working level was not used as it was designed, exposing employees to a fall hazard.  The A-frame ladder was being used in a folded position and not opened according to the manufacturer's specifications.  Observed on or about October 21, 2013.
