29 CFR 1910.146(c)(5)(ii)(C): Employees entered into permit spaces that met the conditions set forth in 29 CFR 1910.146(c)(5)(i) but before an employee entered the space, the internal atmosphere was not tested with a calibrated direct-reading instrument:    (a)  On or about November 19, 2013, the employer had not calibrated the ToxiRAE II PGM-1100 Series Personal Toxic Gas Monitor according to the manufacturers instructions since they had purchased it in October 2013 and was used when employees entered the bag house, which was considered a permit required confined space.      (b)  On or about September 10 and 30, 2013, employees were exposed to entering the bag house with a toxic gas meter that was not calibrated.  The employer did not have calibration records for the original meter that was used when employees entered the bag house, which was considered a permit required confined space.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)
