29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. On the flat roof of the office located inside the facility, a maintenance employee was exposed to a 20 ft. fall hazard while changing an air fliter on a HVAC unit.

Test voltages and the maximum intervals between tests shall be in accordance with Table I-5 and Table I-6. A maintenance employee was using rubber insulated gloves rated at 5000 Volts AC that had not been tested in over 5 years.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed. Note: Personal protective equipment requirements are contained in subpart I of this part. On or about August 30, 2013, a maintenance employee was changing fuses on a bus bar without wearing the appropriate personal protective equipment to protect himself against electric shock and arc flash.

The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. The Company had not documented nor implemented a Energy Control Program consisting of energy control procedures, training and periodic inspections.
