29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    On or about October 31, 2013 and at times prior thereto, employees operated Lian Rou LR-PS-90P pocket spring coilers in the pocket coil department and were exposed to tripping and falling hazards:    a) On the floor at number 1 pocket spring coiler, the 240 volt flexible power cable;     b) On the floor at number 2 pocket spring coiler, the 240 volt flexible power cable;  c) On the floor at number 3 pocket spring coiler, the 240 volt flexible power cable;  d) On the floor at number 4 pocket spring coiler, the 240 volt flexible power cable;  e) On the floor at number 5 pocket spring coiler, the 240 volt flexible power cable;  f) On the floor at number 6 pocket spring coiler, the 240 volt flexible power cable;  g) On the floor at number 7 pocket spring coiler, the 240 volt flexible power cable;  h) On the floor at number 8 pocket spring coiler, the 240 volt flexible power cable;  i) On the floor at number 9 pocket spring coiler, the 240 volt flexible power cable; and  j) On the floor at number 10 pocket spring coiler, the 240 volt flexible power cable.

29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition.  On or about October 31, 2013 and at times prior thereto, employees working in the bonnell department were exposed to slip and fall hazards where water leaking from the roof accumulated on the floor:  a) At the sliding bay door entering into the bonnell department where the border rod machines were operated; b) At the queen and king border rod 6 gauge wire machine and equipment; and  c) At the twin and full border rod 6 gauge wire machine and equipment.

29 CFR 1910.36(b)(1): Two exit routes. At least two exit routes was not available in the workplace to permit prompt evacuation of employees and other building occupants during an emergency, except as allowed in paragraph (b)(3) of this section. The exit routes must be located as far away as practical from each other so that if one exit route is blocked by fire or smoke, employees can evacuate using the second exit route.    On or about October 31, 2013 and at times prior thereto, the employer did not ensure that exit doors were available or at least two exit routes were available in the workplace for emergency evacuation. Employees were exposed to the hazard of entrapment and fire:    a) In the bonnell department where the border rod machines were operated, and  b) In the pocket coiler department.

Stairs or a ramp were not provided where the exit route was not substantially level:    On or about October 31, 2013 and at times prior thereto, in the pocket coiler department, the employer did not ensure that the exit routes were free and unobstructed by items such as but not limited to storage racks/carts, boxes, pallets, stacked materials and wire coil bundles, and equipment.  Employees were exposed to the hazards of entrapment and fire.

On or about October 31, 2013 and at times prior thereto, in the pocket coiler department, the employer did not ensure machine guarding or methods to protect employees from amputation and caught-in hazards where utilized:    a) on the Lian Rou LR-PS-90P pocket spring coilers number one through ten, the installed protective doors were not being closed when the machines were in operation, and    b) on the Lian Rou LR-PS-90P pocket spring coilers number one through ten, the installed protective doors did not have interlock systems to prevent the machines from operating when the doors were opened.    The Sonic Component Systems LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 891977, citation number one, item number 1a and was affirmed as a final order on 06/21/2013, with respect to a workplace located at 1101 N Business 45 Corsicana, Texas.

On or about October 31, 2013 and at times prior thereto, the employer did not ensure that strain relief devices were installed to prevent strain or pull of the electrical conductors and cords, exposing employees to the hazards of electrical shock and fire:    a)  In the pocket coiler department, employees operated ten (10) Lian Rou LR-PS-90P pocket spring machines and wire coil turntables with 240 volt flexible cords with strain and conductors visible, and    b) In the bonnell department, the cable sheathing of a two wire 120 volt electrical outlet system was pulled away from the outlet handy box.     The Sonic Component Systems LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number 891977, citation number 1, item number 4b and was affirmed as a final order on 06/21/2013, with respect to a workplace located at 1101 N Business 45 Corsicana, Texas.
