29 CFR 1910.158(e)(2)(iii): Hose systems were not inspected at least annually and after each use to assure that all equipment and hose are in place, available for use, and in serviceable condition:        a) Pulp dryer area, first floor: On October 28, 2013 and at times prior, employees were exposed to burns and fire hazards associated with a fire hose that was not inspected at least annually or after it was used on October 25, 2013 to extinguish a fire in the pulp dryer. The fire hose had visible damage to the outside of the hose.       b) Pulp dryer area, second floor: On October 28, 2013 and at times prior, employees were exposed to burns and fire hazards associated with a fire hose that was not inspected at least annually to insure that it was in serviceable condition.
