29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation were performed by the customer or user without manufacturers prior written approval:                                                          This violation occurred on or about October 25, 2013, and at times prior thereto, where employees used a Mitsubishi forklift truck, with a metal cage attached to the forks.   The employer, without receiving prior written approval from the manufacturer, used the cage to carry trash from the facility to the dumpster located across the street.
