29 CFR 1926.100(a):  Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  a)  Employees engaged in hoisting operations  and working below roofing operations were exposed to struck-by hazards were not wearing hard hats.  Carlos Amaro dba Amaro Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 682918, citation number 1, item number 1, and was affirmed as a final order on December 11, 2012, with respect to a workplace located at 7527 Myrtle Ave., Chicago, IL 60631.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a)  Employees performing roofing work were exposed to fall hazards of approximately 22 feet.    Carlos Amaro dba Amaro Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 682918, citation number 1, item number 2, and was affirmed as a final order on December 11, 2012, with respect to a workplace located at 7527 Myrtle Ave., Chicago, IL 60631.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
