29 CFR 1910.133(a)(1)  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    Inside the South Plant machine shop, specifically 51 Auto Bulkhead, Retrack Line, employee(s) performing welding work were wearing prescription safety glasses.  Appropriate eye wear would include eye wear that contained a minimum protective shade of ten.

29 CFR 1910.147(f)(2)(i)  Whenever outside servicing personnel were to be engaged in activities covered by the scope and application of this standard, the on-site employer and the outside employer did not inform each other of their respective lockout or tag out procedures:  Outside the South Plant, at the pit below the truck scale, the onsite employer did not inform the outside servicing employer of its respective lockout procedure or ensure that such a procedure was utilized prior to entering the permit space.
