29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: 681 New England Road, Guilford, CT 06437    The 23 foot 6 inch high walking/working surface was not equipped with a fall protection system to prevent employees from falling over the 23 1/2 inch high, 18 3/4 inch wide form wall above the walking/working surface exposing employees to an 18 foot 6 inch fall.    On or about 30 October 2013, an employee was injured after falling over the 18 3/4 inch wide concrete form wall while bending over utilizing a concrete vibrator.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each  employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Location: 681 New England Road, Guilford, CT 06437    The employees who were exposed to fall hazards of 18 feet 6 inches above the ground, while working on a walking/working surface on the side of a concrete form wall, were not fully trained in fall hazards and the procedures to be followed to minimize those hazards.  Hazards such as, but not limited to, procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems; use and operation of guardrail systems, personal fall arrest systems, and safety net systems.
