Location: 681 New England Road, Guilford, CT 06437    The 23 foot 6 inch high walking/working surface was not equipped with a fall protection system to prevent employees from falling over the 23 1/2 inch high, 18 3/4 inch wide form wall above the walking/working surface exposing employees to an 18 foot 6 inch fall.    On or about 30 October 2013, an employee was injured after falling over the 18 3/4 inch wide concrete form wall while bending over utilizing a concrete vibrator.
