29 CFR  1926.501(b)(5):Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:  Location: 245 5th Avenue, Waltham, MA   a) On or about 9/28/22, an employee conducting formwork was not protected from falling 6 feet or more by a personal fall arrest system. An employee fell approximately 14 feet from the wall, partially landing on the footer, and subsequently falling to the ground.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  Location: 245 5th Avenue, Waltham, MA   a) On or about 9/28/22, the employer did not prepare a written training certification for an employee that used a personal fall arrest system, prior to working at heights in excess of 6 feet on formwork. The employee fell from the formwork to the ground below.
