29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: On or about October 30, 2013, inside the house under construction, the employer did not ensure that electrical equipment was free from recognized hazards for the following instances: a) Inside of the framed house and used by employees to operate their power tools, a hand-made electrical handy-box with a single duplex receptacle (15A, 125VAC, 2-pole, 3-wire) bared electrical conductors, it had no cover. b) A 100-foot orange extension cord used to power the Skilsaw Worm Drive Saw (model #HD77M) had damage to the outer insulation. c) A 100-foot red extension cord used to power the Makita 7 ¼-inches Circular Saw (Model #5007E) had extensive damage to the outer insulation.

The employees were exposed to a fall hazard of greater than six feet.
