Employees were exposed to delay in escape, struck by falling load and caught in-between hazards.

29 CFR 1926.651(e): Employee was not prohibited to be underneath loads handled by lifting or digging equipment: On or about October 30, 2013, at the job site located at 10200 Highway 5 North, Alexander, AR 72002, the employer did not ensure employees were safe from falling loads. Employees were working beneath a 1000 pound load suspended from the bucket of the 330LX tracked heavy excavation vehicle. The unprotected employees were exposed to struck by and caught in-between hazards as they worked to set new pipe in the excavation.

On or about October 30, 2013, at the job site located at 10200 Highway 5 North, Alexander, AR 72002, the employer did not ensure employees were protected from falling/rolling materials or equipment into the trench. The excavated soil was placed at the edge of the trench on the west end near the excavator where employees were working. The unprotected employees were exposed to struck-by and caught-in-between hazards.
