29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energy startup or release of stored energy could occur and cause injury, the machine or equipment is isolated from the energy source and rendered inoperative:     a.  1900 Buford Hwy., Duluth, GA (Throughout Shop) - An employee was exposed to sudden energization or start-up of multi-energy source machines.  The employer did not develop and implement a lockout/tagout program with machine specific written procedures for locking out/tagging out the multi-energy machines to safe guard the employee performing cleaning, routine general maintenance and minor repairs, on or about 10/30/13.             a.  Glue Spreader Machine - 120 psi/240V         b.  Pinch Roller  - 120 psi/240V        c.  CNC Router - 120 psi/240V        d.  CNC Saw - 120 psi/240V        e.  Edge Bender - 120 psi/240V        f. 29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:   a.  1900 Buford Hwy., Duluth, GA (Production Area) - Employees were exposed to being struck by flying debris.  Employees were using compressed air to clean the wood debris/shavings off the machines and employees' clothing.  The air pressure gauge measured greater than 30 psi, on or about 10/30/13.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a.  1900 Buford Hwy., Duluth, GA (Production Area) - An employee in the workplace were voluntarily wearing a filtering face piece (dusk mask) for additional protection of being exposed to wood dust.     Information for Appendix D was not provided to the employee, on or about 10/30/13.
