On or about March 4, 2015:   An employee who was temporarily assigned to the packaging area of the facility was cutting 7/16th inch OSB (oriented strand board) while utilizing a Powermatic 10 inch Table Saw, Model: 66TA-SAW, Serial Number: 04120662695, RPM: 3450, Volts: 230, received deep lacerations to his hand.  The employee was admitted to the hospital and had two surgical procedures performed on his hand.  The incident occurred on March 4th at 7:15am and was not reported to an OSHA representative until March 7th at 5:42pm.
