29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) Jobsite, 6022 Glades Pike, Somerset, PA: On or about 11/04/13 employees performing roofing activities on a roof with a slope of 1/12 were not protected from falling approximately 22-28 feet by guardrail systems, safety nets, personal fall arrest systems or a combination of warning line system and safety monitor. The area of the roof were employees were working was approximately 70 ft. x 80 ft. and no warning line system was erected to ensure employees were aware of the work area. In addition, the person who was to be the safety monitor was working with the employees moving materials and not engaged in monitoring the employees' movements.

The failure to utilize fall protection systems indicated that employees failed to retain the requisite understanding and skill necessary to prevent exposure to falls.
