Facility - On or about October 31, 2013, the written lockout tagout procedures for employees who service equipment on site were not adequately developed in that specific testing requirements to verify the effectiveness of lock out tagout devices were not identified. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
