29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    Media /Wash Plant - On or about April 11, 2016, authorized employees (supervisor, lead man, pickers, and other employees) were required to remove guards to clean conveyors (M1, M2 and M3). Each authorized employee did not affix a personal lockout device to the electrical disconnect(s) associated with conveyors M1, M2 and M3 before beginning the cleaning of the conveyors. Employees were thereby exposed to the hazards associated with a inadvertent startup of the equipment.

29 CFR 1910.147(c)(7): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:    Media / Wash Plant - On or about April 11, 2016, an employee was caught in a conveyer and died as a result of his injuries.  Prior to his injury, the deceased employee was not trained properly in lockout procedures.'

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  Plant #3 - On or about April 11, 2016, the employer had not documented through a written certification that periodic inspections had been performed on the lockout / tagout procedures in the company's energy control program. Employee(s) were thereby exposed to the hazards associated with the inadvertent start-up of the conveyors.
