29 CFR 1910.111(b)(8)(i): Hose used in ammonia service did not conform to the joint Agricultural Ammonia Institute - Rubber Manufacturers Association Specifications for Anhydrous Ammonia Hose.  a)  Building 34:  The anhydrous ammonia feed hose to the Cyclone Furnace # 451 did not comply with the specifications for durability given by the above consensus groups.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from liquid chemicals, acids or caustic liquids, or chemical gases or vapors.  a)  Building 34:  Employees who were designated to investigate the source(s) of an anhydrous ammonia leak around the Cyclone Furnace #459 did not wear either chemical splash goggles or faceshields with safety glasses underneath to protect their eyes in the event of a sudden release of anhydrous ammonia.

The violations listed below have been grouped together as a single violation because they involve related hazards that may increase the potential for occupational illness resulting from exposure to a toxic material.   29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, and at least annually thereafter.  a)  Building 34:  Maintenance Men were not given an initial fit test to the Scott full facepiece respirator, which are on stand-by in the event of ammonia releases.

29 CFR 1910.134(h)(3)(ii)(A): Respirator inspections did not include a check of respirator function and the condition of the various parts including, but not limited to the cartridges, and canisters or filters:  a)  Building 34:   The Scott full facepiece respirators were equipped with ammonia canisters that had expired in January 2013.
