29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope: a) Exterior Front, Top of Bay Window - A carpenter was observed standing on a 6 inch wide top plate while installing short trusses for the roof on a bay window, and the lifeline used in the personal fall arrest system was wrapped around an upper window mullion and the snap hook was attached back into the lifeline, exposing the carpenter to a fall of 8 feet to a lower roof level, on or about 11/05/13. For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(d)(6)(v): A snaphook not of the locking type was engaged to an object which was incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself: a) Exterior Front, Top of Bay Window - A carpenter was observed standing on a 6 inch wide top plate while installing short trusses for the roof of a bay window and the snap hook used to secure the lifeline to a window mullion would only allow half of the line into the throat of the snap hook, exposing the carpenter to an 8 foot fall hazard, on or about 11/05/13. For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
