This condition exposed employee to tip-over and falls from elevation.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: On or about November 4, 2013, at the worksite located at 201 South Izard in Little Rock, Arkansas, south side of roof, a midrail was not provided on the front of the Haulotte Biljax 4527A lift basket that employee used to access and descend from roof. This condition exposed employee to falls up to approximately 32 ft. from elevation.
