29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees engaged in operating armoring and braiding machines were exposed to amputation hazards in that: 1. In the armoring area, armoring machines numbered 0,1,3,4, and 5 had missing guards in the area of the outgoing product, approximately 5 inches in length and 5 inches in width. 2. In the braiding area, the lower part of the braiding machines numbered 3, 4, 5, 6, 7, 8, 9, 10, 11, and 12 were missing guards.

29 CFR 1910.37(b)(1): Each exit route was not adequately lighted so that an employee with normal vision can see along the exit route. In the Armoring area the path of travel for exit routes were not adequately illuminated, in the event of an unforeseen occurrence such as a power outage.
