29 CFR 1910.212(a)(3)(iv)(f): The point of operation of power saws whose operation exposed an employee to injury were not guarded:    a)  BDDW, 3450 Salmon Street, Philadelphia, PA, First Floor, Veneer Area- The Langzauner LZ 5/2E veneer saw had an unguarded circular saw blade that passes through the table area directly in front of the operator exposing the worker to severe lacerations or amputation injuries. Observed on or about November 6, 2013.    b)  BDDW, 3450 Salmon Street, Philadelphia, PA, First Floor, Metal Shop- The Wilton Cold Saw, Modello Willy, had an unguarded circular saw blade exposing the operator to severe lacerations or amputations while operating the metal saw.  Observed on or about November 6, 2013.

Observed on or about November 6, 2013.       b) BDDW, 3450 Salmon Street, Philadelphia, PA, First Floor, Rough Cut- The 220 volt 3 phase power supply to the Power Matic 12 inch table saw, was a flexible cord run from a box on the wall along the floor under wood in the working area of the operator. This condition created the potential for cutting or abrading the cord and exposing conductors.  Observed on or about November 6, 2013.

The cut connectors were left taped together in a wad, rather than removing the unused service and plugging the unused opening in the box.
