29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:   a) BDDW Studio, LLC: Employees operating machines such as planers, millers, and sanders were exposed to tripping hazards as debris fell below and around the equipment the employees were operating. The employer did not ensure that all working areas were kept free from accumulation of tripping hazards. Observed on or about March 6, 2014.

29 CFR 1910.134(c)(1): The employer did not establish and implement a written respiratory protection program with worksite specific procedures for voluntary respirator users:  a) BDDW Studio, LLC.: The employer did not develop a written respiratory protection program for those employees who voluntarily wore tight-fitting, half-face, negative pressure, elastomeric respirators. The employees donned the respirators while seasoning and staining wood products. Observed on or about March 6, 2014.

The employer did not ensure that employees were provided with  medical evaluation prior to donning the respirators. Observed on or about March 6, 2014.
