29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Benjamin Steel Company, Inc. - Mansfield, Ohio: On or about November 5, 2013, the employer did not assure that employees, who accessed the top of stacked/stored bundles of steel, were protected from falls hazards. An employee(s) who had climbed onto a stack of steel bundles in order to rig an adjacent bundle was exposed to a fall hazard of 4.5 feet.
