29 CFR 1904.10(a): Where an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level was 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, the employer did not record the case on the OSHA 300 Log.       Roselle Paper Company, Inc. - Production areas such as, but not limited to the Sheeting Department and Sewing machine area.:  Two (2) employees annual audiograms revealed the employees experienced a work-related STS in hearing, and the employees' total hearing level was 25 dB or more above the audiometric zero (averaged 2k, 3k, and 4k Hz) in the same ears as the STSs.  The Employer did not record the cases on the 2013 OSHA 300 log.        a)      Production areas such as, but not limited to Sheeting Department - An employee experienced a work-related STS in the left ear based on the report of their annual audiogram.  The employee's total hearing level in the left ear was 41.6 dB above the audiometric zero (averaged at 2k, 3k and 4K Hz).    b)      Production areas such as, but not limited to Sewing machine area - An employee experienced a work-related STS in the left ear based on the report of their annual audiogram.  The employee's total hearing level in the left ear was 53.3 dB above the audiometric zero (averaged at 2k, 3k and 4K Hz).        Condition noted on/or about December 18, 2013.

Production Areas:  The Employer did not supervise employees correct use of hearing protectors.  Employees were observed improperly wearing their hearing protection such as, but not limited to Howard Leight and Bilsom T2 ear muffs by wearing them over their hats.  The ear muffs cups rested on the hat, which breaks the seal to cover the outer ear.       Condition noted on/or about December 18, 2013.

Employees wore hearing protector such as such as, but not limited to Howard Leight and Bilsom T2 ear muffs, but were not refitted and retrained in the use of hearing protectors.       Condition noted on/or about December 18, 2013.
