29 CFR 1926.152(a)(1): Containers other than approved metal safety cans or DOT approved containers were used for the handling and use of flammable liquids in quantities of 5 gallons or less:     a) A five (5) gallon plastic gasoline container was provided for employee use to handle gasoline for the purpose of fueling a portable generator.  The can was not an approved safety can because it did not have flash arrestor screens nor a self-closing lid.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:     a) Employees were provided with electrical tools and equipment connected by cord and plug to a Champion 6000 watt portable generator, having 7000 watts of start-up power, without the use of ground fault circuit interrupters or an assured equipment grounding conductor program.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):       a) Front Elevation - Employees were exposed to the hazard of falling while performing carpentry work from an 8/12 pitched roof having a ground to eave height of approximately 25 feet without the use of fall protection.       b) Rear Elevation - Employees were exposed to the hazard of falling while performing carpentry work from a 4/12 pitched roof having a ground to eave height of approximately 25 feet without the use of fall protection.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
